Does Dr. Gallup see children and adult patients?

Yes, we care for patients of all ages; including children, adults, and the elderly.

What insurance companies does Village Family Practice have an agreement with?

We are contracted with most major plans, including Medicare, BlueCross BlueShield, United Healthcare, Aetna, Wellpath, Cigna, Humana, and Medcost. Our billing staff is available to tell you more about your plan and potential financial responsibility, if you need more information.

How long does it take to obtain an appointment?

We have built in same-day appointments for those requiring urgent care. Even for non-urgent visits, such as wellness exams or school physicals, an appointment can frequently be made on the same day or within a few days.

How do I have my medical records sent from another practice?

To release records from another practice, we need your written permission. Please ask our receptionist for a medical release form, and we will take care of the rest. It will usually take two or three weeks for the records to be received.

What do I need to bring with me to my appointment?

Please be sure to have your most current insurance ID card, a photo ID, and a list of your current prescriptions. If you would like us to request medical records, then don't forget to bring the name and number of your other physicians.

How should I request a prescription refill?

Please call your pharmacy to request the prescription at least 5 days before you run out of your medicine. They will contact us directly.

What is the best way to contact the office in case I am having a non-urgent problem following a visit or procedure?

Please call the office directly and ask for a nurse. We will return all calls received by 4:00 pm by the end of the business day.

What about contacting you after hours and on weekends?

For an emergency situation, please go to the nearest urgent care or hospital. For other urgent, but not life threatening situations, please call the office and select the phone menu option to "contact the physician on call". Our answering service will take your information for Dr. Gallup; and he will respond quickly. For non-urgent questions (i.e. an appointment request or prescription refill request), please phone our office during normal business hours or send the request through our website. We will process and respond within 24 hours.

What are some examples of an emergency medical situation?

An emergency generally involves an acute illness that is potentially life threatening. Individuals experiencing any of these conditions should go to the nearest emergency room or call 911. Although not a complete list, here are some examples of true emergency situations: blood in urine; chest pain; stroke symptoms; trouble breathing; sudden, severe pain, such as a headache or stomachache; head or back injuries; bleeding or vomiting that won't stop; loss of consciousness; poisoning; major burns and cuts; choking; a suspected fracture after a fall or accident.

Will I need to get a referral before seeing another physician?

To determine whether you need a referral before seeing another physician, view the terms of your insurance plan. This information is listed on the insurance card; or it can be obtained by calling your insurance company. ( Please be aware that the patient is ultimately responsible for assuring that the proper processes are completed for such visits.) Of course our office will work with you to obtain authorizations and/or complete documents needed for approval from your specific insurance plan.
